Help Locked out of phone, stuck at password screen

Very strange thing has happened twice over about 3 weeks. After the 15 minute timeout I have set (per company EAS policies), twice it's happened where it just does not accept my password at all and I have to do a CM recovery to get back into the phone. First time I was running Skyraider 2.5.2 then it happened yesterday on Virtuous 2.7.

They have the pattern unlock disabled by security policy and only PIN and Password are available, so I chose PIN.

Anyone have this happen at all?
