Root Lost carrier (virgin)

After trying CM12, I restored back to rooted zv5. However, when I did, I lost 4g. I downgraded to zv3 and allowed the phone to updated itself to zv4 and then zv5. No luck. Then, I followed the instructions here: http://androidforums.com/threads/weird-issue-when-downgrading-to-z3.889100/ but no luck.

Any suggestions would be greatly appreciated.
