Root Memory card corrupted after rooting phone

ever since i rooted my phone, ive been having problems with my evo memory card. i formated it and everything tried different roms but same thing happens whenever i take a picture,listen to music, or anything involving the memory card.
all apps crash except for the camera which says cannot save files to the SD card due to insufficient file permissions. can anyone help me?

i had something like this happen. make sure fat or fat32 type of format, from the pc gui, and it will take longer but dont do the quick format..do the longer one..it may take 10-15 min but it finally formatted it correctly for me.
