Has anyone used this? My wife just busted the screen on her phone. Do they charge to fix or replace my busted phone?

So no one has replaced a phone because they broke it. I would like to know what I'm going to get charged. Is the insurance a ripoff and I will end up buying the LG Motion.

$39 and need to make an insurance claim. Her phone is 1 year old to date $60 + $39. The bad part is the phone is not replaced on the spot it comes in the mail. I canceled the insurance and bought the LG Motion.
