Ok, I tried regularly installing it, and I tried putting it in system/app and setting the permissions, rebooted but it didn't come up. :'(

Hummm.. are you using non-stockbased rom such as Kitkat or CM11?

In order to attempt fix back-camera issue on CM11 rom, I tried to install this stock camera app but failed, it seems like it's missing necessary files and permissions. Wondering if you have same issue here.

Did it work on other ROMs? It may be that your camera isn't supported.
