Help Missing App Icons and Widgets on Homescreens

I have a problem where app icons and widgets are automatically removed from my homescreens. Is anyone else having this problem with their Rezound?
 
In order to help you we need more information. My phone has none of these problems so describe the issue in a bit more detail.
