Root Missing Visual VoiceMail

I'm running superclean 2.2 ADW Launcher EX and I cannot find Visual Voicemail by Verizon. I received a text saying "to reinstall click on the icon," but there is no icon to be found. I also tried looking through Titanium and it wasn't listed there (at least nothing apparent).

Any help would be greatly appreciated!

Update:

Found the program in Titanium but it is crossed out and when I click on it it doesn't give me the option to restore. I found the data but for no reason the apk...
