I solved it with apps2SD - see previous post(s).

Also, I believe 4.4.2 doesn't have the problem of using the volume serial number to reference the SD card.

I have tried formatting the new (128GB) SD card on a PC but nothing seems to work on it.

Try using SD Formatter; if even that won't work then the card is likely FUBAR and should be returned as defective.
