If you've rooted phone and custom rom may be your recovery mode need upgrade sometimes that also it can be the cause so if you have ROM manager it's easy to upgrade recovery mode but you gotta have WiFi when you upgrade.

Are you using the LG online updater tool, or using the KDZ_FW_UPD tool?

Also, make sure the KDZ_FW_UPD is in emergency mode. Copy-paste the output of KDZ_FW_UPD.
