Help Phone crashes after being restored every time.

Hello, My HTC Droid Incredible 2 crashses every time I restore it and the restore is not always successful. It is often stuck on the bootloader.
When i turn it on the following is displayed:

I have tried every option on BOOTLOADER.
RECOVERY and FACTORY RESET just leave me with a red triangle.
SIMLOCK displays:
-5: SD FAT32 init fail !!!
Process done, reboot device?
<VOL UP> Yes

I have wiped the cache partition and done a factory data reset and nothing happened.

The phone cannot be detected by my computer.

I already tried that too. It sometimes works but then crashes soon after.
Apps force close and then the whole phone crashes and goes to the bootloader.
 
Sounds like the internal flash memory is corrupt or bad... might want to just get it back to stock and try to get a replacement from VZW if it is under warranty, if it isn't and you don't have the insurance or extended warranty, perhaps try CM7.2 Stable ROMs, it couldn't really hurt anything at this point.
 
It is not under warranty and it hasn't been rooted. It is on stock. I will try that when I get the chance to...
Any other suggestions?
