Help phone goes to disc drive mode

My phone always goes into disc drive mode mode even when I select charge only. Worse, it goes to disc drive mode even when I charge using the wall charger, which means I can't access anything from my SD card when I charging my phone through the wall charger or my car charger. Any idea how to fix this? Thanks.

Happens to me sometimes too.

Did you try rebooting?

That usually works for me

I did a battery pull but it is still happening. thinking about doing a hard reset but hesitating because of all the apps I have bought or downloaded.
