Help Phone Vibrate Icon missing?

Anyone missing the icon at the top? On vibrate it shows the no sound icon.

If I put my phone on silent mode with a toggle widget, it displays the no sound icon (speaker with "X"). However if I silence it by holding the power button and selecting silent mode that way, it displays the vibrate only icon (tilted box with lines).
