Sure thing, it's the Minimal MIUI Go Apex Theme in the Market. I think it was $2 or something, well worth it though, the icons are very sharp.

The icon pack is called "Monochrome"
The background is the AudioGlow Live Wallpaper (it costs $0.99 I think)

Disable notification bar and dock; set up a 5x7 grid in your favorite launcher. I set a swipe-up gesture to show the dock, swipe-down to display notifications.

I'm guessing it's an Ubuntu theme. I know Go Launcher has one.

It's adw ex with beautiful widgets, zooper widgets, WiFi signal widget, and the status bar hidden. 10x5 screen layout with icons at 70%. Theme inspired by ubuntu android os. With metro icons and cobalt inferno cm 10 theme.
