I'm having an issue with my first gen Incredible when I add music to it via my Macbook. I've maxed out the phone with mp3s but when I remove them (using the Mac interface) this doesn't free up any space. Sure enough the songs/albums are gone from my phone when I check it, but when I reconnect the phone to the computer it still says I still have the same, tiny amount of available space.
