problems after softward upgrade

[nancyh1990]

My phone is bone stock. No rooted....
Yesterday, after upgrade, all my widgets show: "problem loading widget".
Now my bluetooth on/off widget is gone. Also bluetooth is in applications. I can manually turn on/off bluetooth when I go to menu>setting>Wireless & Networks>Bluetooth. But I had a widget from application. How can I get that back. Thanks

remove the widget and re-add it.

I did remove the widget.
Can you show me how to add the widget when Bluetooth is not shown when I go to application?
