Root Problems trying to activate de-rooted phone.

My old phone was an OM. I'd rooted it, then had it returned to stock as I was getting a Motion and planning on giving the OM to a relative.

Reprogramming the OM on the phone with the metro rep (*228) failed twice even though we did everything correctly. At their suggestion I took it into a store.

They tried hard reset, line refresh, everything. Nothing worked. The girl found the problem eventually, the MEID DEC # in the phone menu was different than what appeared on their screen. Ended in a string of 0's, which they said it wasn't supposed to.

They gave me a troubleshooting ticket to come back 4-5 days later.

I'm thinking that having rooted and uprooted my phone caused this somehow. What else could have? If so am I screwed?

What, if anything, can I do?
 
Also I should mention that after returning it to stock, I'd gotten it stuck in fastboot when I pressed "restore factory settings" or whatever it is, and I had to have the guy who had rooted it get it out of fastboot.

Of course I made no mention of any of that rooting stuff to metro.
 
Also-I forgot this. They said it was showing that my relative's service had been successfully transferred to my old OM, it just won't make calls, and an error message is heard when placing calls.

"LA Zero One. Your phone is not authorized to receive service at this time..."
