Help proximity sensor failure after repair

I had a shattered screen on my droid X - I purchased a new screen from ebay and installed it based on instructions found on youtube.

After I installed the new screen and put the phone back together, the proximity sensor is malfunctioning. Every time I make a phone call the screen goes blank and stays there. I have to remove the battery to stop the call.

Any ideas where the proximity sensor is and how I could make it stop working?!

The proximity sensor is immediately to the right of the earpiece, there is a small rectangular cutout for it.

The proximity sensor is only activated during a call, so if you somehow bumped it or moved it so that it's no longer facing forward or no longer positioned behind directly behind the clear rectangular cutout, it will be active all the time and will shut off when a call is placed.

You could also have damaged it or damaged its communication back to the board, but shifting it would be pretty easy and would cause the symptom you are describing.
