If I use ROM manager to do so, will it format my SD card in the process? I don't have a computer, so backup is difficult..

Don't know about ROM manager, never used it, but if it does any kind of SD card re-partitioning or formatting, it will erase and overwrite all the data on there.
If your device has USB OTG, you can easily backup from the SD to a USB stick or external USB SD reader/writer. No computer required.

Yes, as it has to delete the existing single partition, create two new ones, and format those for use.
