Just have to say, epic thread title!
In an answer, unless it's someone I don't care to talk to, I generally answer ASAP. Don't see a reason in letting them sweat over it.

Heck, I thought the current title was the one people were commenting on - I'm not sure I want to know what the original was! 
I try to answer straight away, unless there's a good reason not to answer then. Only deliberate delay is that I check caller id before answering, but that only adds a fraction of a second. Time to dig the phone out of my pocket is usually the largest delay.
Time I spend listening to other people's ring tone? Longest is usually when phoning home and nobody can find a handset (problem with all of the phones being wireless - they migrate!). Then I hang up on the 7th ring before it can go to voicemail, wait a few seconds, then try again - usually get a quick pickup second time
