Root Rooted and now what??

Rooted my Evo last night and it said it was successful.

I rooted so that I could use the hotspot without paying sprint & now I still cannot get the hotspot to work.

Do I have to install a custom rom in order to get the hotspot working? & How do I know for sure if I rooted correctly?

You don't use sprint hot spot app. You have to use an app from the market. Like Wireless Tether.

You can use the sprint hotspot app if you want, but you must flash a ROM that has it unlocked. There are several out there. Myn's Warm, for example, does this. There are several more out there. Of course, MissJennell is correct in that android wifi tether, from the market, is much easier to use if you don't want to bother flashing ROMs.
