Help S2 Freezing while sleeping on the charger

It all began recently.

Sometimes while my Galaxy S2 (i9100) is charging (wall charge or usb - doesn't matter) and i try to unlock it - it doesn't respond or light up at all. I tried to call myself to see if it wakes but it doesn't do absolutely nothing - not even a "blink" or sound. I have to hold up power button until it reboots to fix it.

After I force reboot it - the battery seems charged up as normal.

This issue occurs at random and only while it's charging, so i don't really know what causes it. I'm using the same charger/cable as always. I don't have bumps on my battery either.

It looks like it freezes while sleeping (that sounded weird haha), so it needs force restart.
What do you suggest me to do?

NOTE: I'm rooted.
 
The obvious question to ask is have you installed any apps recently that can affect the power use of the phone? Maybe a battery saver or security app? If so, uninstall it and see if that clears up the problem. You could always try a factory reset, if it's an app problem, but I don't know that it's worth the extra work to backup and restore everything at this point.

Are you sure the phone is not simply off? All Li-ion batteries come with some sort of overcharge protection circuitry. It is supposed to stop the charge cycle when specific voltages are reached. However, if the circuit goes bad, it might cut off power to the device completely as if the battery were removed.

I'd try a replacement battery to see if that fixes the problem.

No, i don't use battery savers. The only "battery" app I've installed is Battery Calibration to clear the batterystats.bin when changing ROMs.

Yes, I am 100% sure the phone isn't off regardless the lack of response from it.
