SD Card disconnects

I've encountered a very strange problem.
Sometimes, seemingly at random, the SD-card disconnects itself, and a message on the drag-down menu tells me it's safe to remove it.

For instance, this has happened while I was using apps on the SD-card - Which suddenly then doesn't seem to exist on the phone anymore.
Rebooting only helps sometimes.

Is there a solution to this and has anyone else encountered something similar?

I just had that problem. Eventually my SD would not even click back into place, after taking it out. I had to send to be replaced for free. I assume the SD mechanism wore out prematurely.
