Help SD Card does not mount anymore

Before I proceed I will say I have researched this site for an entire weekend trying everything imaginable under the sun for this issue and nothing has worked.

I have a PC with windows 7 and have never had issues. I had to do a factory reset on my Incredible because it refused to get out of roaming and could not pull a regular signal. After the reset, I would plug my phone into my PC like usual and it just charges. The drop down notification does not exist anymore so I cannot choose the option to mount the sd card. I have tried my friends Incredible on the same PC and it worked fine. I tried a different computer with a different OS and it still did not work. I tried everything including power cycling and doing a ballerina of removing cards and power cycling and plug into pc sequences to no avail. I deleted the USB drivers for the Dinc on my PC and still no luck. I tried the debug on/off thing in the settings. I did 2 factory resets and still nothing.

Am I missing anything? Is it possible the hardware is just giving out?
 
have you tried reformatting your sd card?
have you tried another known working sd card in your phone to narrow your problem down to the phone or sd card?
 
Go into Settings > Connect to PC and check ask me.

It may be as simple as it is defaulting to charge only because you accidentally hit don't ask in the popup.
 
Are you able to see it on the phone with any file explorer?

When you look in Settings > SD & Phone storage is it mounted?
 
Both are mounted, and I am able to dig through the files on the sd card with Astro. I have to take the card out and plug into the laptops sd card slot just to transfer files back and forth. So its not the card. I am able to play music off the card and access files through the phone and PC.
 
It's not the USB cable, I tried three cables that worked before. I am just going to get Verizon to send me a replacement...great.
 
I know you said that it's not the cable, but bear in mind that I had to use the actual cable that came with the phone before I got any luck myself, a standard micro USB will not work right for some reason. Probably a warranty issue, as you say, but just something to keep in mind.
