Help SD Card Issue

So when I go to SD & phone storage my SD Card shows up like this

SD card
Total Space
Unavailable
Available Space
Unavailable


Then Mount and Format are grayed out (can't be selected) what is the problem?

Yes it's all the way in. I took the phone apart and made sure it was.

Went to listen to some music that I have stored on my sd card tonight, and when I couldn't find them I went into the memory and noticed that mine set something similar to yours...I don't remember exactly how it was worded but it was acting like I didn't have a sd card in the phone (which I have of course since I first got it out of the box)...I turned the phone off and did a battery pull and then turned the phone back on... as it was booting back up it gave the little symbol up at the top and said "preparing sd card"...now I can see all my music and when I go to the memory settings it shows what's in the card now...hope this isn't a bug that's going to re-occur...
