Help SD Card Issues?

I'm not sure if anyone else has had this issue with their original Droid, but I have. And it's getting frustrating. Every 3 days-1 week my SD card is wiped. It says that it's not compatible and needs to be reformatted. The first time was a small fluke, not too much of a nuisance, but It happened right when I was leaving the state. Then it happened again two weeks later, then it happened this morning.

Does anyone know if this is a card or phone issue? Thanks!

Probably the SDCard. I'd take it out of the phone after powering down, then using a card reader to back it up to your computer, and finally format it using a full format. I started having issues about 7 months ago, and when I finally did this, it took three tries to fully format it - but after that I've had not a single issue.
