SD Card occasionally disconnecting/reconnecting

Over the past week or two, my phone has spontaneously disconnected my SD Card. Then, it issues warnings for missing apps/files/pics (those I had moved to SD). Finally, it reconnects on its own and finds the missing stuff. I don't know if the update caused this or if it never reared its head before as I couldn't store apps on my card. Anyone else facing this?
 
This happened with my previous phone (Bionic). Folks here suggested my SD card was probably dying, and that I should (1) back it up ASAP; and (2) replace the card.

I did this. No further issues.
 
It might help to identify the MicroSD brand/type. I am in the process of returning a Sandisc Class 10 card which has had issues similar to what you mention (and others).
