Help SD Card

All of a sudden my phone does not recognize my SD Card. I put the card into a card reader and my computer won't recognize it either - do SD cards randomly go bad?? If so, is there any way to get the information off the SD card?

Is it the original card or a replacement card??

It is not the original card - it's a card I've had for a while with all my media on it and have used it in the Evo for the last 7 months without any problems.
