I am stock 4.1.1 verizon rooted, but with 4.2 camera/gallery app. I was doing some romming, system modding, but then restored nandroid... dont know when it happened exactly, but my thumbails were getting swapped between by Camera and Screenshot folders...
I cleared cache and data for Gallery in Settings > Apps, and gallery/camera then both FC'd as they said they couldn't access sdcard storage, so I rebooted, and the issue was fixed... weird. Just sharing this in case anyone else finds this useful info.
