Speakerphone not working

Anyone else have this problem? I have an otterbox, but it doesn't cover anything up. Sometimes people can hear me and sometimes they can't.
 
I have an otterbox as well, but have not encountered this problem. Is there a setting for the microphone volume or on/off?
