Storage useage?

I am using the stock desire rom which has been rooted and has a2sd+ enabled with dalvik on ext. I understand that my phone internal storage will still go down but what is using the space?
1- The actual apk be on the ext, correct?
2- Is the app cache and app data still stored in the internal memory?

If this is correct and i add up all the app cache and app data (not the apk size but actual data) size's shouldnt this equal the used internal storage space? I have tried this and have 48MB of space that i cant account for.

Would sms's, emails etc not show up as data storage usage?
