Help Strange unlock problem

Just got an LG G3 and enjoying it after my 2+ year old Bionic, but I'm having a strange problem on unlock. It doesn't happen often, but sometimes when I unlock the phone, a blank, orange G+ screen comes up. I hit home to get to the home screen when this happens. I've searched this and other forums and can't find this mentioned anywhere.

I don't have a G+ icon on my home screen, or on any screen for that matter. I haven't been able to recreate the problem on purpose no matter how I swipe to unlock. If I go to apps and open G+ from there, the screen is black, not orange. I can't find any setting anywhere that would launch G+ by default.
 
No, haven't tried that. I haven't used it on this phone yet, but I do use it, which makes me realize, it shouldn't be blank regardless. If I crank up G+ on my tablet it populates with my feed. On the phone, whether it opens from unlock or I start it, it's blank, with no access to a menu. I will try to force stop first, as I don't want to disable it, but would like to know why this is happening.

As I unlocked the phone to check this, it happened again, but a black screen this time.

So I go in to Force Stop and scroll down, disabling notification, etc. and there is a panel that says, "Launch by Default. You have chosen to launch this app by default for some actions," with a Clear defaults button. Well, no I haven't, but I hit the Clear Defaults button and now this area is greyed out.

After that, I started it from the icon and it starts normally. Maybe problem solved?
 
Nope. Happened again today. Swipe to unlock and I have a blank, black G+ screen. Soon thereafter, I do a swipe to unlock and I have a completely blank, black screen with nothing on it but notifications. I could swipe down to look at them, but nothing else worked. Did a restart to get back to normal.
 
Under Settings / lock screen / customize, have you cleared all of the check marks? Just making sure that nothing is interfering with the lock screen default settings.
 
Only had missed calls and power button locks checked. I will try with those unchecked. Since it's only sporadic and I haven't been able to reproduce on demand, it's hard to know what works.
