Help The process com.android.media has stopped

Does anyone else see this message (don't know if this is the exact message, but it's close) fairly frequently on their phone? I've been getting it on the stock 4.0.2, 4.0.3 and 4.0.4 images that are out there.

I'm unlocked and rooted, but on the otherwise stock images. Right now I'm on the 4.0.4 with latest radios image.

I'm completely stock and been getting it lately. Not sure what it is.

I had this pop up on me at least once a day. Called Verizon who contacted Samsung and they couldn't figure it out so they sent me out another and have had no problems with this one. This was a couple of weeks after launch so they might have figured it out by now.
