I think that is ADW launcher, it has the ability to show an app drawer by sliding the menu button up or down.

ADW launcher has the ability to hide the icon names, and I have no clue as to what the widget is.

The icons are probably from an iPhone icon pack.

The widget is most likely the switchpro widget.
