Help Unable to format sd card

All,
Need some help.
I have a SanDisk 16GB micro SD card, that I am unable to format.

I have tried formatting the card on the device, and on a PC using a card reader.

No matter how I try, the card does not seem to format and erase.
All my previous documents/pictures/music files are still there.

Does anyone know what might be causing this?

Does the phone somehow lock the card so that it cannot be altered?
And if so, is there a way around this.

BTW, many times, after the phone has been up and running for a while, I will get a status message indicating that the SD card is mounted for read only.

Any help would be greatly appreciated.

Thanks.

Could be faulty SD Card. Mine went out yesterday, files all showed up but it wouldn't read or write to the card.

When I put it in the phone it would show up, but if I tried to view a pic stored on the card, it would go away and say, SD Card unexpectedly removed. When I put it in the PC and tried to copy off, it gave errors that it couldn't read the card.

I hear the SD Cards go out quite often.

which brings up a question I have, anyone know of a good utility to try and save data off a faulty SD Card?

Definitely sounds like a bad sd card...
Recuva from the makers of ccleaner is a great data recovery solution
