Root Unrooted incredible App/contacts issue

I rooted my Incredible (unrevoked) worked perfect for 2 weeks all of a sudden massive issues. lost contacts, changed/erased pictures for contacts, apps (stop unexpectedly/force close)

I then unrooted the phone back to the 2.2 with the zip file on the SD card method. again the phone worked perfect for a week or so then right back to the same exact issues listed above.

Im at a total loss on what to do any help would be appreciated, right now the phone is barely usable with the contacts going missing ect ect. PLEASE HELP!

Could be a hardware problem, or you are installing an app that is causing the problem.
