[Verizon] How do I wipe everything and get back to factory settings

I've had a terrible time since I tried a 4.2 ROM and messed everything up somehow. I've backed my Internal Storage (or SD Card) to my computer and I just want to wipe EVERYTHING and get back to factory settings so I can start over. I've tried to do a Hard Reset and nothing is working. I would just like to somehow get it back to the point it was at right after I unlocked and rooted.

Any help is very much appreciated as I am at a loss for now!

For the benefit of our readers, can you please explain how you fixed it? Someone else may have the same issue and your fix could be priceless to them
