Help Verizon LG G3 on TMoblie LTE Issues

Hello,

So I've spent the last 3 days looking over every thread I could find on getting this phone set up. So we got SMS and Calls working fine, but it won't connect to the internet or MMS. It shows the 4G LTE on the top and it shows that it's connected in the settings and also in LTE Discovery. But it just says offline or no connection when we try to go anything related to the internet. It also has the unknown SIM card on the status bar as well. Only thing I can think of is getting a new SIM. If anyone has any suggestions please reply.

I've tried the following APNs.

Has the phone been completely unlocked from Verizon? Master Subsidy Lock, MSL I believe it's called.
