[Verizon] Returning broken phone

I cracked my screen the other day and i got a replacement phone from the insurance that verizon uses and my phone is currently unlocked and rooted. how do i go back to stock? do i unroot it first or relock it first?
i do have a backup of when b4 i first flashed any roms.

Several different ways to deal with this...

If you have a Nandroid backup of your stock (pre-/un-rooted) setup, then you could restore that. You'd still have a non-stock (custom) recovery which you'd have to take care of if you were worried about that particular aspect, but you could flash the stock recovery in fastboot before re-locking your bootloader.

Don't forget to re-locked the bootloader--that's probably the one thing VZW might look at/for.

I don't think it would be necessary to flash it back to stock if it's broken...but i'm sure you've already sent it back:p
 
If you paid the deductible, then don't worry about it. I'd only return it to stock for a warranty replacement
