Help VOIP calling issues

I'm trying to use Google voice to make a voip call. I've used it successfully before, but now I'm having a weird issue. Whenever I put the phone to my ear, the signal quality drops and becomes impossible to have a conversation. I'm using sipdroid and can see the loss % shoot up. But if I use speakerphone or hold the phone away, the quality is perfect. Any ideas why?

Ok well I figured out that its the screen auto shutoff that seems to kill the call. I found an option in sipdroid to keep screen on, and now it works fine. Except now it burns extra battery and my face pressing on the live screen has accidentally turned a call or two off.
Now, I wish i knew why the screen shut off kills it. WiFi is set to never sleep so besides that nothing obvious comes to mind...
