Help Wallpaper issues since last update

Anyone else having wallpaper issues since the last update on the incredible. I actually did a factory reset and I still have times when I turn on my phone after being off and the wall paper reverts back to the blury lights wallpaper. If I re-select one of my themes, it comes back. I have also lost my custom ring tone at times.

Is your wallpaper stored on the sd card?

Tried both SD Card and Phone memory. Originally my SD card went bad so I moved the wallpaper to the phone.
