It doesn't get hot, but it's very warm at the top by the earpiece. My Eris and Incredible got hot at the battery so you didn't feel it on your face like this.
I notice it warm and much warmer on a few occasion. On one, I left the phone in my car and went to the gym for an hour. When I came back the top of the phone was noticeably warm to hot. Maybe my car and my phone were chatting since BlueTooth was active on the phone and the car was still probably putting out some kind of signal.
Its pretty much the screen generating the heat. Just like your TV warms up, being a small HD samoled screen, this gets pretty toasty, especially with increased brightness.
