Left pants pocket for me, and it's always been that way. I'm right-handed, and the wallet goes in the right pocket, keys in the back pocket. Call me odd. :D
 
Holster on my belt when I am out.. When I'm in the car it sits in a cradle, so I can use navigation and music without having to hold it :)
When I'm at home/work it's usually on my desk or a table somewhere..

That works for many of my shirts. Good point.

It stands up quite well in many others. That pocket is the best place for me for a lot of reasons: I can hear it readily in a noisy area is the best reason.

That danged vibrate is not helpful every time; I was missing calls with the device on my hip in the holster.
 
In my purse, no case. Sorry, OP, looks like your purse-related assumption was wrong. :p

There's a phone-size zipper pocket mine stays in, screen facing the inside of the purse, with a (you're going to laugh) folded-up maxi pad for cushioning/further protection from the jumbled contents of my purse! :p If anyone knows if any cases that both protect the screen AND let you leave the phone in the case to use it I'm all ears. Lol
 
Eris - left front pocket.
Keys, cash, junk - right front pocket.
Wallet - right back pocket.
