Think it's called "SilentMode On/Off" by Curvefish. Toggles from full-on to vibrate to silent modes. You can also name the widget on your home screen anything you'd like. Pretty cool. Wish the icon was the same size as the HTC control widgets ... but then again, I'm particular.
