Yes it does. If you go into your wireless and networks settings, it's called Wimax. You should be able to add a button to the Notification Power Widget in the CM7 Settings to toggle it on and off, but again, it's referred to as Wimax.

You can also download the 4G widget for EVO from the android market. I haven't had any troubles at all while using the 4G toggle widget
