the path is correct, but I think the build is overwriting one of the files with its own

I had two mistakes
one that isaacj87 pointed out, one file was overwiting the other
the second mistake was I was taking the same file from two different places

I am coming across an issue...
The first picture is how my app drawer looks when I first install cm12. The 2nd picture is what my app drawer keeps changing to, by its self. Each time it does it, I have to wipe and reinstall.
Does ANYONE know why this keeps happening?

That's just it. I haven't installed anything other than the ones that Google has saved, but all of those install directly after flashing the OS.

I haven't installed any launchers. I really like the stock experience... But I really want my app drawer to stay like it is after a clean flash.
I just tried doing a dirty flash, and it didn't change back
