Go to Settings/Apps and look for the icon on the list. The name of the app will be next to it. You probably did that already, but I thought I'd suggest it just in case. I've encountered an app once or twice that wasn't showing up in Apps. I don't know what that means other than it's a virus.
we get this question everyday. the problem is that 99$ of the time the phone does not belong to the poster. also there are millions of apps out there each wuith their own icons. so unless it is something popular like youtube or facebook you will only get guesses from us here. and most of these go unanswered.
Actually, it could also be because the app was installed in Xposed or an Xposed variant. I think that happened on my phone. It probably doesn't apply here, but I just thought I'd mention it.
