Root [Help] Cannot Mount SD Card (CWM 4.0.1.5)

Well the title says it all i cannot mount the sd card for some reason and atm i can only access CWM because i was going to put on another rom so i formatted everything. Please help. The sd card i have is a 4gb sandisk micro sd card that is in fat32. Any thoughts/ideas? The error message for anything related to updating/installing/restoring is E:/ cannot mount sd card. I do have a backup of the original phone software but cannot restore because it requires an sd card

You don't have the Stock uSD card lying around somewhere?
Have you tried reformatting you uSD card using your PC? Make sure you choose to restore to default setting. Then transfer your backup files onto it as well as the ROM and Gapps making sure you include the Android and .android_secure folders.

Put sd card back into phone and power up into the recovery. In the recovery menu go to install zip from sd card/select zip from sd card and see if you can see the folders as well as the ROM and Gapps. If no, try going to "mounts and storage" and select unmount sd card.
If yes, go back into the menu and wipe and format only those things necessary for a ROM install making sure you do not select format sd card or ext.

If this doesn't work try another uSD card.
