Thought I would give you guys a heads up on what I noticed. I was laying in bed today trying to organize things on my phone. I noticed there was a TERRRRRIIIIIIBBBBBLLLLLEEE lag, and that my phone kept going into failsafe mode. I thought it was a custom lock screen I had installed, but I did a wipe and restored a NAND recovery of before that and it was still failing. Then I POSSIBLY pinpointed the culprit.... My phone was on the charger, and the battery was getting warm because of its charge, so it kicked it into FS mode. I know, you guys probably are thinking "duh, that's obvious", but if you didn't know that, and you ever have this problem, then here is a possible explanation.
Plus, the silicone covering I think was keeping in the heat. Took it off and its cooling down now.
Guys, just a quick FYI: while charging my phone here at work, I decided to pull-up System Panel and watch the Temperatures gauge while it charged.
The temp started at 32.4 C and quickly rose to 40.2 C within 15 minutes. I had my failsafe temp set to 41 C and realized that it wouldn't take very much to get it to climb past this limit.
No big deal, I adjusted the limit to 43 C and took the silicone sleeve off of the phone and the temp has held steady at 40.2. Anyway, lesson here just reinforces namu1107's post that charging will easily bump the phone temp up by 8-plus degrees (and that's just while sitting, being charge--nothing else being done on the phone). Your mileage WILL vary.

edit: silicone sleeve being off allowed the temp to drop to 37.7 C while still charging...removal of sleeve seems to have made the biggest difference (i.e., not holding all that heat in)...charge is at 91% and temp is 35.4 C
