Dock apps are as follows:
+ is App Drawer
Next two are both Browser, and the last two are phone
If you swipe up on the first icon spot for the browser it goes to ESPN. Swiping up on the second icon spot brings me to this forum

I've got transparent icons for every spot on the dock. The first one with the arrow up and the plus sign launchs my app drawer, while the next two open the browser. The last two open my phone. Swiping up on either of the browser spots will launch a bookmarK.
As far as the icons, I got them off xda, called Kovicon Hybrid Blue,.

tool bar should be in your default widgets. I believe it is called power control. The battery widget looks like batterylife which you can get from the market.
