Root SD card cluttered and full... can I format it?

I have had my Incredible since June and have had the same 16GB SD card in since day one. Since then, I have downloaded/uninstalled hundreds of apps, put music, games and other files directly on the SD card, rooted the phone, used ROM manager to download countless Roms and mods, made quite a few nandroid backups, and the list goes on. Well of those 16GB I have 1 left. Everything is cluttered and unorganized and there isn't that much music on it, so a lot of the stuff is probably just junk.

My question... can I format the card and start over? What would happen? I assume my phone would still be rooted as it is now, and if I booted to recovery with a clean SD card (+ one ROM to boot to and perhaps a backup file just in case), can I simply flash the ROM and go on with my life? What important information is on the SD card that I'm forgetting? I've found my few favorite ROMs and developers, and I am going to try Google's cloud music sync, so I want the clutter gone for good. What is my best course of action?

For the record, all of my contacts are stored in my google account and some data for games is backed up through Titanium (is that a server or is that on the SD?)
 
Titanium backups are on the SD. At minimum, I'd save that folder and one nandroid backup just in case. There may be some apps that are dependent on a reference file also. To be on the safe side, move the contents of the card to your PC. Then move back what you really think you need. If you miscalculated, you still have access to all your previous stuff.
